Question on etiquette for massage parlors

I'm new to using massage parlors and need some advice. What is the proper way to ask for extra services? Come right out and ask or wait until the provider brings it up? Also, what is the a typical "tip" above the house charge to pay for these services? Will the tip vary depending on the length of the session or the particular service. For example, if I want a 90 minute massage rather than 60 minutes, I know the house charge will be higher. What about the tip for additional services?
I'm in Rochester if the city makes a difference in the answer.

Plenty on info in the reviews posted but having PA will help giving you far more info.
If you're only there for extras, just book 30 mins. If you really enjoy the massage too, then do an hour. Some girls are terrible at massages, some are great. You just won't know until you know. She'll start to tickle your yam bag, and you'll know it's on. Then she'll probably just ask you what you want. Just tell her. It's much less complicated than you think. Go somewhere that it'll be a sure thing. Check for reviews.

Premium Access. It lets you see the details of the reviews. I don't have it yet but looking forward to it. You get it by doing a Review. You can pay for it too. The best etiquette I have found is that you find out what the going rate is for the place you are going and offer it up front. You usually don't have to say anything. They don't really want you to. For example in the NYC boroughs it is somewhere around 3 bills for an hour. Just plunk it down at the start and get ready. But not everywhere offers everything. An hour is standard in NYC. 30 minutes isn't going to be much cheaper and they will rush you. I don't know how things go in Upstate. But I will definitely be finding out!
Thought PA was Pennsylvania.

Getting back to the question, for a first timer, usually the provider will make a non-verbal signal. And yes lay down a heavy tip before.
