Depending where you are located look for a plastic or orthopedic sugeon that is board certified and trained in the sub specialty of hand surgery.

If you havent already tried physical therapy you might want to try a rehabilitation center, splints, non steroidal antinflammatories, etc.

When I am seeing special doctors, tests, and certain exams, and I tell them that I don't have insurance. I get 20% to to 30% off my bill. I had to do cancer biopsy, and I pre-paid $5500 up front. I got $300 back after the biopsy.

Have you looked into acupuncture? I've had issues with tendinitis in both hands and my right elbow for the last 10 years or so. I was skeptical about it until my SIL convinced me to give it a shot. Not only has it relieved my tendinitis, it's also helped with some chronic knee pain I've recently been dealing with.
