A member can accumulate points and ban himself/herself automatically, or a staff member can go to the profile and ban it from a control panel that is not available to the general public. It's quite simple.

He either didn't follow instructions (and got points), someone confirmed his identity, the staff confirmed multiple profiles, or was sharing computers with a provider.

Anyhow, it was only a matter of time for him to acquire enough rope to hang himself.
