Here's a much easier approach.
Instead of concerning yourself with feedback you receive from some silly
questionnaire, focus your efforts on making sure your 'client-friend' has
difficulty making it out to his vehicle. Concentrate your efforts on making him
noodle-legged. and then you will have NO doubt as to how the encounter went.

There are a few things you can implement to help decrease the chances of a bad date:

1. Remember that you cannot make everyone happy...all of the time. Be selective on whom you choose to meet. Consider their personality...how they post...who they choose to meet...how he communicates his ideas to you...and how he responds to your PMs.

2. Be sure to be HONEST with your DESCRIPTION in your profile. If you're a 40DDD-30-38 and weigh 170#s...don't state you're only 36DDD-25-36...and weigh 145. That's false advertising! Embrace your true beauty...when you do...you don't need to fabricate your description!

3. Post REAL pictures of yourself. (which you do)
4. Don't take on too many commitments and burn youself out.
5. Lastly...be true to yourself and and don't be FAKE...be considerate of others...and be respectful.

These are just a few tips for you to consider. They are proven factors in helping to ensure you're doing your best to be a reputable provider.

ATX you read my mind. They're surveys are fairly simple, just yes and no answers and clicks. Easier to do it online than in person on paper. Several restaurants and I think HEB uses it. Plus they usually give a discount code for a free drink or appetizer after you complete it....hint,hint. . Lol jk.

You can use survey monkey to set it up! It's just a matter of getting them to fill it out :-)
